The reason the shoe is such a deadly insult in middle-eastern countries dates to the time when the streets were far, far filthier than they are today. so the bottom of the shoe is considered unclean. this is the same reason you take your shoes off at the door in many cultures.

IOW, in terms of symbolism there's pretty much an assumption of doggie doo at the very least, and maybe even human doo-doo as well.

Picking a shoe is insulting in many cultures, Kruchev did that at a UN meeting when protesting against some issue and he took his shoe and kept hitting the desk.
Sardar Abdul Rub Nishtar did it too when he was upset at Patel in a session prior to partition.

I've already send the weblink of this 'shoey' Iraqi greeting to those I know (and don't wish to know any more) on the other side of the Atlantic, with THIS added:

"Watch the mass-murderer president being presented with shoes in Iraq. May the demons of Hell shower him with countless 'chittars' until Hell itself freezes over!"

I was impressed at the way he was able to duck to avoid the first hit, for real, he has his reflexes intact even though the brain is hibernating, maybe for that very reason...
